Why Wisdom Teeth Cause Problems — and When Removal Is Necessary

Wisdom teeth are the last molars to erupt — usually between ages 17–25. They're evolutionary leftovers: our jaws have become smaller, but we still grow 32 teeth. The result: most people don't have room for wisdom teeth.

When wisdom teeth don't fully erupt, they become "impacted" — stuck under the gum, angled wrong, pressing against neighbouring teeth. This causes:

Severe pain, often radiating to the jaw, ear, or head

Gum infection (pericoronitis) with swelling, bad taste, and fever

Damage to the neighbouring second molar (decay, root resorption)

Crowding and shifting of front teeth

Cysts and (rarely) tumours forming around the impacted tooth

In most cases, the only effective treatment is surgical removal. The longer you delay, the harder the extraction becomes — roots harden, complications increase, recovery extends.

Complete Dental Care for Every Age

Simple Extraction

Wisdom tooth fully erupted, normal angulation. Removed with regular extraction technique. 15–20 minutes. Recovery: 2–3 days.

Surgical Extraction (Impacted)

Wisdom tooth partially or fully under gum/bone. Requires small incision, possibly bone removal. 30–60 minutes. Recovery: 5–7 days.

Complex Surgical Extraction

Severely impacted, close to nerve, or unusual angulation. May require sectioning the tooth. 60–90 minutes. Recovery: 7–10 days.

How Wisdom Tooth Removal Works at 3D-DDS

01

3D CBCT Assessment

We take a 3D scan to see the tooth's exact position, angulation, root anatomy, and proximity to the inferior alveolar nerve. Critical for safe extraction.
No surprises during surgery. We know exactly what we're dealing with.

02

Anaesthesia & Sedation

Local anaesthesia for most cases. Conscious sedation available for anxious patients or complex multi-tooth removals.
You're completely comfortable throughout. No pain, no anxiety.

03

Precise Surgical Extraction

Dr. Swathi Devulapally — a trained oral surgeon — performs the extraction. For impacted teeth, a small incision and bone removal allow safe, controlled removal.
Surgery, not "extraction." Performed by the right specialist.

04

Recovery & Aftercare

Sutures (dissolvable in most cases). Detailed aftercare instructions: ice, soft diet, medication schedule. Follow-up in 7 days to check healing.
Most patients return to work in 2–3 days for simple cases, 5–7 days for complex.

Common Questions About Wisdom Tooth Removal

Is wisdom tooth removal painful?

The procedure itself is completely painless under anaesthesia — you feel pressure, never pain. Post-op discomfort for 2–3 days is normal and managed with regular painkillers. Most patients are pleasantly surprised at how manageable it is.

Can all four wisdom teeth be removed at once?

Yes — for healthy patients, removing all four in a single session is common. We typically do this with sedation. Recovery is consolidated into one period rather than four separate ones.

How long is recovery?

Simple extractions: back to work in 2–3 days, full healing in 7 days. Surgical impacted extractions: back to work in 5–7 days, full healing in 2–3 weeks.

What if my wisdom tooth isn't painful? Should I still remove it?

Not always. Asymptomatic wisdom teeth that are well-positioned, easy to clean, and not pressing against other teeth can sometimes be left alone with regular monitoring. We'll honestly tell you if removal is unnecessary.

Are there risks?

Minor risks include: dry socket (painful but treatable), infection (rare with antibiotics), prolonged swelling, and very rarely, temporary numbness if the tooth is close to a nerve. With 3D CBCT planning, serious complications are extremely rare.

What can I eat after extraction?

Soft, cool foods for 24–48 hours: dahi, kheer, smoothies, mashed potato, ice cream. Avoid hot food, hard food, spicy food, straws, and smoking for 5–7 days.
